Texas DSHS does not routinely manage or maintain Yellow Fever-specific vaccination records, including International Certificates of Vaccination or Prophylaxis (ICVP or “yellow cards”). Patients who received a Yellow Fever vaccination should first contact the physician or clinic that originally administered the vaccine, as that provider is the primary custodian of the immunization record.
ImmTrac2 (Texas Immunization Registry)
If you have consented to have your or your child’s information stored in ImmTrac2, your Yellow Fever vaccination may be included in your official Texas immunization record if the vaccinating provider reported it to the registry. Not all providers report Yellow Fever vaccinations to ImmTrac2.
Important information about ImmTrac2 records
- ImmTrac2 was established in the late 1990s; individuals born prior to this time may have incomplete records.
- Immunization records are only available for individuals who provided ImmTrac2 registry consent for their information to be stored in the registry.
- Adult records may be limited if an Adult Consent Form was not completed before age 26.
- If a Yellow Fever vaccination was not reported by the provider, it will not appear in ImmTrac2.
