Conscientious Exemptions

In accordance with Texas Administrative Code §97.62, Texas law allows for an exemption from immunizations for reasons of conscience, including religious belief. To claim an exemption due to reasons of conscience, a student's parent or legal guardian must request, sign, and submit an official DSHS affidavit form to the child's school. The affidavit is valid for two years.

Note: Students who haven't received the required immunizations for reasons of conscience may be excluded from school in times of emergency or epidemic declared by the Texas Commissioner of Health.
