Mailing and Shipping Information
Laboratory Services Section
A post office box address is now in effect for DSHS. By December 1, 2007, all mail must reflect the new mailing address: PO Box 149347 with zip code 78714 and Mail Code 1947, as detailed below. The new mailing address does not affect previously established PO Boxes (newborn screening, clinical chemistry specimens, etc), though Mail Code 1947 should be added after "Laboratory Services Section." Note: If you have a different PO Box number, it probably falls under the "previously established" category; please check with your department contact for verification.
