    E-mail data requests to HIV/STD Program - This email can be used to request data and statistics on HIV, TB, and STDs in Texas. It cannot be used to get treatment or infection history for individuals, or to request information on programs and services. Please do not include any personal, identifying health information in your email such as HIV status, Date of Birth, Social Security Number, etc.

    For treatment/testing history, please contact your local Health Department.

    For information on HIV testing and services available to Persons Living with HIV and AIDS, please contact your local HIV services organization.

Tax Transcript and Certificate of Non-Filing Instructions


To request an Income Tax Transcript Request or Proof of Non- Filing Request, visit the IRS website [IRS].

If you are a first timer user, click “Get Started”. If you are a returning user, enter your user ID and click “Login.”

Step 1: Enter the person’s first name, last name, and email address.

Step 2: Check email and enter security code. Security code expires in 30 minutes.

Step 3: Enter social security number, date of birth, filing status (single, head of household, etc.), and address.

Step 4: Complete confirmation of credit

Step 5: The system will ask what the tax transcript is for, select either “income verification” or “health care.” This will direct you to “return transcript.”

Step 6: Select the most recent year and then print the transcript. The form is created as a PDF. This means the information could be saved electronically by the agency.

 

*UPDATE (7/1/2015): The IRS system for requesting tax return transcripts online is currently unavailable. Tax return transcripts can be requested by mail by completing Form 4506-T (PDF) [IRS] or by calling 1-800-908-9946. IRS instructions for obtaining tax transcript. [IRS]
