Transition Workgroup

The Texas Title V Transition Workgroup, formerly known as the Purchased Health Services Unit (PHSU) Transition Team, meets quarterly via conference call to share knowledge, implementation strategies, and best practices. Providers, youth and young adults, parents, caregivers, as well as representatives from health care plans, hospital and university systems, and local community organizations are invited to participate. To promote a collaborative approach to the provision of transition services for youth in Texas with disabilities and special health care needs and their families.

Strategic Plan

Beginning in 2013, the Texas Title V Transition Workgroup members gathered to create the workgroup’s first strategic plan. An exploration of the workgroup’s strengths, weaknesses, opportunities, and challenges informed the selection of strategic priorities. Next, stakeholder and family member recommendations assisted in the creation of specific goals, strategies, and objectives. The resulting Five Year Strategic Plan is a product of the workgroup’s efforts, and is intended to support members as they strive to further the mission and vision of the TTVTW.
