May 22, 2017, 12 month probated suspension for violating EMS Rules 25 TAC §§157.36(b)(6), 157.36(b)(3), 157.36(b)(4), 157.36(b)(7), 157.36(b)(28), 157.36(b)(30), and 157.36(b)(31) related to accepted and responded to a call that required a Bariatric EMS Unit with ACLS capabilities when in fact it did not, lacking proper training and equipment for the care and transport of a bariatric patient which resulted in the stretcher tipping over and patient spent 20 minutes on pavement, and falsified and/or forged the patients and/or family members’ signatures on the patient care report.

Carnes Professional Services LLC DBA Legacy Emergency Medical Services DBA Legends EMS, College Station, TX. October 17, 2016, revoked for violating EMS Rules 25 TAC §§ 157.11(m)(30), 157.11(q)(2), and 157.16(d)(12) related to failure to provide the Department with requested information regarding an investigation/inspection, the Department unable to conduct EMS compliance survey and ambulance inspection as the gate was closed on inspector and on the second attempt the Provider cancelled day of inspection,  the Provider ceased operations and failed to notify the Department.

Diserens, Brandon S., Gatesville, TX. September 23, 2016, revoked for violating EMS Rules 25 TAC §§157.36(b)(2), 157.36(b)(6), 157.36(b)(23), 157.36(b)(25), 157.36(b)(28), and 157.36(b)(30) related to inappropriate sexual contact with a female approximately fifteen years of age, inappropriate sexual contact with a child or children approximately seventeen years of age or younger, and arrests for sexual assault of a child and continuous sexual abuse of a child or children.

February 2, 2017, assessed a $6,000.00 administrative penalty for violating EMS Rules 25 TAC §§157.11(b)(6)(V)(iii), 157.11(m)(15), 157.11(n)(2), 157.16(d)(5) and 157.16(d)(10) related to deficient renewal application and submitted to the Department a falsified letter of credit. 
Dudley, Stephen W., Shelbyville, TX. March 24, 2017, 3 month suspension for violating EMS Rules 25 TAC §§157.36(b)(7), 157.36(b)(28), and 157.36(b)(30) related to starting an IV without medical direction and beyond scope of certification. 
Easterling, Jacob W., Fredericksburg, TX. October 6, 2016, reprimand for violating EMS Rules 25 TAC §§ 157.36(b)(30) related to possessing medications without having a physician’s prescription or appropriate medical authorization.
Fisher, Eric R., Malakoff, TX. March 8, 2017, 36 month suspension; 3 months actual suspension followed by 33 month probated suspension for violating EMS Rules 25 TAC §§157.36(b)(2), 157.36(b)(14), 157.36(b)(19), 157.36(b)(28), 157.36(b)(29), and 157.36(b)(30) related to tampering, adulterated, misappropriated and/or self-administered Fentanyl from employer.

January 23, 2017, 24 month probated suspension and assessed an $82,000.00 administrative penalty probated for violating EMS Rules 25 TAC §§157.11(m)(2)(A), 157.11(m)(2)(E), 157.11(m)(2)(F), 157.11(m)(2)(G), 157.11(m)(3), 157.11(m)(7), 157.11(m)(8), 157.11(m)(9)(B), 157.11(m)(12), 157.11(m)(21), 157.11(m)(29), 157.11(m)(32), 157.16(c), 157.16(d)(1), 157.16(d)(12), and 157.16(d)(19) related to deficient and/or inadequate Medical Director involvement, patient care and documentation of quality assurance, credentialing of employees, record security/retention, complaint management, training material and documentation of training provided, and failure to provide information to the Department regarding an investigation.

Lawrence, Joshua L., Dripping Springs, TX. July 27, 2017, 5 day suspension for violating EMS Rules 25 TAC §§157.36(b)(5), 157.36(b)(6), and 157.36(b)(30) related to taking pictures of accident scene, patients,  and victim and then posting on social media with inappropriate statements which was offensive/abusive to family/friends of accident patients and victim.
Lazbuddie Volunteer Fire Department, Inc. DBA Lazbuddie EMS, Willis, TX. March 23, 2017, assessed a $3,750.00 administrative penalty for violating EMS Rules 25 TAC §§157.11(g)(9), 157.11(m)(1), 157.11(m)(3), 157.11(m)(4), 157.11(m)(12), 157.11(m)(32), 157.16(c), 157.16(d)(14) 157.16(d)(19), and HSC §773.050(a) related to staffing an EMS ambulance vehicle with a person that had an expired DSHS issued license and/or certificate, and failure to develop, implement and enforce policy, procedure and protocols. 
Licerio, Gabriel, El Paso, TX. December 2, 2016, revoked for violating EMS Rules 25 TAC §§157.36(b)(2), 157.36(b)(6), 157.36(b)(22), 157.36(b)(23), 157.36(b)(26), 157.36(b)(28), and 157.36(b)(30) related to an arrest for unlawful restraint, two counts of violation bond/protective order, two counts of assault causes bodily injury/family member, and resist arrest.  Indicted on two counts of violation of protective order assault/stalking and failure to notify the Department of arrests and indictments.
Longoria, Norma J., Pharr, TX. August 11, 2017, reprimanded for violating EMS Rules 25 TAC § 157.36(b)(7) related to failure to follow medical director protocols.
Mallette, Caleb M., North Richland Hills, TX. May 19, 2017, revoked for violating EMS Rules 25 TAC §§157.36(b)(2), 157.36(b)(3), 157.36(b)(4), 157.36(b)(6), 157.36(b)(7), 157.36(b)(28), 157.36(b)(30), and 157.36(b)(31) related to defibrillating AFib patient several times, failure to follow medical director protocols, falsified patient care report and failure to disclose treatment to receiving ED physician or nurse.

October 14, 2016, revoked for violating EMS Rules 25 TAC §§ 157.36(b)(2), 157.36(b)(14), 157.36(b)(19), 157.36(b)(23), 157.36(b)(28), and 157.36(b)(30) related to misappropriated, tampered, adulterated, and/or self-administered several ampules of morphine from employer without having proper authorization from a physician’s prescription and/or employer’s medical director, and arrest for possession of a controlled substance, and failure to notify Department of an arrest.
