The Medical Home Workgroup serves as a forum for members to
share goals and objectives, report on their organizations’ progress, and
increase knowledge of the philosophy and effectiveness of medical homes.
Members of the MHWG include representatives from health and human service
agencies, private and group practices, health care plans, hospital and
university systems, local community organizations, and parents. To join or learn more about the

To enhance the development, and promote the principles of the
Patient-Centered Medical Home model within the state of Texas for all children
and youth including those with special health care needs.
All children and youth in Texas, including children with special
health care needs, will have a medical home that provides accessible,
family-centered, continuous, comprehensive, coordinated, compassionate, and
culturally competent services.
History and Strategic
The Medical Home Work Group (MHWG) was formed in 2003 after participants
of an American Academy of Pediatrics conference spearheaded efforts to
encourage Texas providers to establish medical homes. The MHWG was formed to
share information, facilitate progress, and coordinate efforts to promote
person-centered medical homes. The group furthered DSHS’ efforts to advance
medical homes for children and youth with special health care needs in the
state of Texas.
A strategic plan was developed in 2006 and revised in 2010. In 2014, the MHWG
defined its mission and vision, and began work on revamping its strategic plan
and adding specific, measurable goals.
